DE 1101ID: Unemployment Insurance Application (paper)
DE 1101ID is the paper version of the application for California unemployment benefits. Most people apply online in myEDD, which EDD calls the fastest way, but you can print this form, fill it in and fax or mail it. It is also used to reopen a claim.
Last checked: · Form revision: DE 1101ID Rev. 6 (6-22)
- Who uses it
- People who cannot or choose not to apply through myEDD. EDD offers four versions: worked in California; worked in California and another state; military service; federal employment.
- When you get or need it
- You print it yourself from EDD's Forms and Publications page (fill-in forms). It can be printed and completed only in English or Spanish; versions in other languages are samples for viewing. America's Job Centers of California help complete it at no cost.
- How to submit it
- Fax to 1-866-215-9159, or mail to EDD, PO Box 989738, West Sacramento, CA 95798-9738 (extra postage required). Both are printed on the form's last page. You can also apply by phone at 1-800-300-5616.

What each part means
Labels are quoted from the form; the explanation is EDD's own meaning in plain words, not advice on how to answer.
- Q1 Work outside California in the last 18 months
- Say whether you worked in another state or in Canada in the past 18 months, and list the state(s).
- Q2 Social Security number, EDD Client Number, other SSNs
- Your Social Security number as the Social Security Administration issued it, an EDD Client Number if you have one, and any other SSNs you have used.
- Q3–Q5 Names
- Your full name, whether it matches your Social Security card (and the card name if not), and any other names you have used.
- Q6–Q9 Personal details and earlier claims
- Birth date, gender, written and spoken language preference, and any California UI or Disability Insurance claim in the last two years with dates.
- Q10 Driver license or ID card
- Issuing state and number of a driver license or ID card; if you have neither, explain how you look for work and get to work.
- Q11–Q14 Phone and addresses
- Phone (with a TTY or California Relay option), mailing address, and residence address if different (no P.O. Box).
- Q19 Employment and wages in the past 18 months
- For each employer: name and mailing address, dates worked, total wages, how you were paid, full or part time, weekly hours, and whether it is a school or public/nonprofit employer. Missing information delays or denies benefits.
- Q20–Q21 Other employers; Alternate Base Period
- List extra employers on a separate sheet. Say whether you want EDD to try the Alternate Base Period if wages in the standard base period are too low.
- Q25 Your very last employer
- Last day worked, gross wages for the last week, employer name, mailing and physical address, phone, supervisor, and a brief reason you are no longer working there.
- Q26–Q30 Strike, school work, return to work, union
- Whether you are out of work because of a strike or lockout; school or public/nonprofit work and recess details; whether you expect to return to a former employer or have a definite start date; and any union or trade association membership.
- Q31–Q36 School or training, availability, self-employment, officer status
- School or training dates and hours; your availability for immediate full- and part-time work in your usual occupation; self-employment plans; corporate or union officer or major stockholder status; elected official status.
- Q37–Q40 Pensions, workers' compensation, vacation or severance pay
- Pension details, expected workers' compensation (carrier, case number, dates), and vacation, severance or in-lieu pay with amounts and dates.
- Q41 Citizenship or work authorization
- Whether you are a U.S. citizen or national. If not, whether you are USCIS-registered and authorized to work, with your document details (A-number, card number, I-94, passport, expiration).
- Attachment D: Disaster Unemployment Assistance
- Complete only if you are unemployed because of a recent California disaster: disaster type, county of residence and work, and your situation.
What EDD warns about
- Answers must be true; EDD says penalties may apply, and an incomplete application can delay or prevent the claim.
- The English PDF is coded DE 1101ID, not DE 1101I. Use the version that matches your situation (California only, California and another state, military, or federal).
