- DE 1101ID: Unemployment Insurance Application (paper)
DE 1101ID is the paper version of the application for California unemployment benefits. Most people apply online in myEDD, which EDD calls the fastest way, but you can print this form, fill it in and fax or mail it. It is also used to reopen a claim.
- DE 4581: Continued Claim Form (paper certification)
DE 4581 is the paper Continued Claim Form you use to certify for unemployment benefits every two weeks. You answer eligibility questions, report any work and earnings, and sign under penalty of perjury. EDD mails it; most people certify online instead.
- EDD biweekly certification questions explained
Every two weeks you answer a short set of questions in UI Online (or on the paper form) to get paid. This page lists the questions as EDD words them today and what EDD says each one means. It explains the question, not how you should answer: your answers must be true and are made under penalty of perjury.
- DE 429Z: Notice of Unemployment Insurance Award
DE 429Z is the notice EDD mails after your claim is set up. It shows your claim dates, your weekly and maximum benefit amounts and the employer wages EDD used. EDD states it is not a final decision on your eligibility.
- DE 1080CZ: Notice of Determination
DE 1080CZ is EDD's written decision on your eligibility. The notice explains the facts and the law EDD relied on and ends with your appeal rights. EDD's forms list calls it a Notice of Determination and/or Ruling.
- DE 1000M: Appeal Form
DE 1000M is the Appeal Form for disagreeing with an EDD decision, such as a denial or an overpayment notice. The appeal goes to the California Unemployment Insurance Appeals Board. A signed letter also works if you do not have the form.
- DE 2501: Claim for Disability Insurance (DI) Benefits (not unemployment)
DE 2501 is EDD's paper Claim for Disability Insurance (DI) Benefits, part of State Disability Insurance (SDI). It is for workers who cannot do their regular work because of an illness, injury or pregnancy that is not work-related. It is not an unemployment form.
- Form 1099-G: Tax form for unemployment benefits
EDD issues Form 1099-G by January 31 each year, showing the unemployment benefits it paid you. Unemployment is taxable on your federal return. EDD says you do not have to report it on your California return.
